Zusammenfassung
Der inkomplette Berstungsbruch resultiert aus Kompressionskräften, die auf die Rumpfwirbelsäule einwirken. Er ist der am häufigsten zu diagnostizierende Typ eines Berstungsbruchs an dieser Lokalisation. Selten ist er mit neurologischen Beeinträchtigungen vergesellschaftet, jedoch müssen im Rahmen der Diagnostik höherwertige Verletzungen ausgeschlossen werden. Im deutschsprachigen Raum wird der inkomplette Berstungsbruch überwiegend operativ versorgt, wofür dorsale, ventrale sowie kombiniert dorsoventrale Verfahren zum Einsatz kommen. Unter bestimmten Voraussetzungen kann auch die konservative Behandlung zu einer adäquaten Ausheilung führen. Da die evidenzbasierte Datenlage zum Outcome nach wie vor insuffizient ist, basiert die Wahl der operativen Therapie derzeit auf den Erfahrungen und dem Ausbildungsstand des Operateurs. Als Hilfestellung zur Entscheidung dienen die kürzlich von der Arbeitsgemeinschaft (AG) Wirbelsäule der DGU (Deutsche Gesellschaft für Unfallchirurgie) veröffentlichten Therapieempfehlungen. Fernziel ist, im Rahmen von prospektiv randomisierten Studien die Datenlage zu verbessern, um eine optimale und strukturierte Versorgung des inkompletten Berstungsbruchs auf nationaler und internationaler Ebene zu etablieren.
Abstract
Incomplete burst fractures resulting from compression forces are the main entity of spinal burst fractures. A neurological deficit in correlation with incomplete spinal burst fractures is scarce but it is important to exclude type B or C fracture within the scope of radiological diagnostics. German-speaking surgeons prefer to operate on this type of spinal fracture, whereas under special conditions conservative management might be a treatment option as well. Surgical treatment options are stabilization techniques using isolated posterior, isolated anterior or combined posterior/anterior approaches. Due to the insufficient evidence regarding the treatment of incomplete spinal burst fractures, current treatment is based on the surgeon’s knowledge and level of education. Recently published treatment guidelines for spinal fractures of the DGU Working Group on Spine Surgery might support surgeons regarding their decision process. The long-term objective is to establish national and international treatment guidelines based on randomized studies with a high level of evidence.
