Zusammenfassung
Aufgrund des steigenden Lebensalters nimmt die Zahl proximaler Humerusfrakturen, die ganz überwiegend konservativ behandelt werden können, stetig zu. Das operative Vorgehen beinhaltet entweder eine Platten- oder eine antegrade Verriegelungsnagelosteosynthese. Bei der Plattenosteosynthese hat die Verwendung winkelstabiler Implantate konventionelle Platten weitgehend abgelöst. Auf die spezifischen Komplikationen dieser neuen Implantate wird hingewiesen, Strategien zu deren Vermeidung werden aufgezeigt. Trümmerfrakturen des Humeruskopfs sollten im höheren Alter endoprothetisch versorgt werden. Die funktionellen Ergebnisse nach inverser Prothese im Vergleich zur Kopfprothese sind im eigenen Krankengut deutlich günstiger. Langzeitergebnisse, insbesondere zur Standzeit inverser Prothesen, stehen allerdings noch aus, sodass endgültige Empfehlungen noch nicht ausgesprochen werden können.
Abstract
Because of increasing life spans, the number of proximal fractures is also increasing. Most can be treated conservatively. If operative treatment is required, internal fixation can be performed by plating or interlocking nailing. In plating, the use of plates with angular stability is currently preferred to conventional plates. Possible complications of these new plate systems and interlocking nails, along with strategies to avoid them, are presented here. In older patients with comminuted fractures of the humeral head, we prefer endoprothetic solutions. In our patients, the functional results using a new inverse prosthesis were better than those after implantation of a humeral head prosthesis. However, long-time results are still lacking, so a final recommendation is not yet possible.
