Zusammenfassung
Hintergrund
Die Hüftarthroskopie ist ein etabliertes Verfahren in der Behandlung orthopädischer Krankheitsbilder, wie z. B. dem femoroazetabulären Impingment. Inwieweit sich dieses Verfahren sinnvoll im Rahmen der Behandlung von Frakturen und Frakturfolgen einsetzen lässt, soll im Rahmen dieses Artikels anhand der Sichtung der aktuellen Literatur und eigener Erfahrungen in einem überregionalen Traumazentrum beschrieben werden.
Material und Methode
Es erfolgte eine Literaturrecherche. Die in der Literatur dargestellten Möglichkeiten werden zusammengefasst; ergänzend werden die eigenen Erfahrungen dargestellt. Abschließend werden die Anwendungsgebiete der Hüftarthroskopie sowie deren Nutzen und Risiken evaluiert und diskutiert.
Ergebnisse
Die Hüftarthroskopie lässt sich mit akzeptablem Aufwand und Risiko in der Frakturversorgung einsetzen. Allgemein bietet sie bei allen Verletzungsmustern die Möglichkeit, Begleitverletzungen zu adressieren. Hierzu zählen freie Gelenkkörper, Knorpelschäden oder Risse des Labrums. Bei der Azetabulumosteosynthese erlaubt sie die Entfernung von Interponaten und das Sicherstellen einer stufenlosen Reposition der Gelenkflächen. Ein wesentlicher Vorteil liegt dabei in der Visualisierung des nicht einsehbaren Gelenkraums. Bei posttraumatischen Bewegungseinschränkungen können minimal-invasiv weichteilige und knöcherne Bewegungshindernisse therapiert werden.
Schlussfolgerung
In Ergänzung der Reposition und Osteosynthese kann die Hüftarthroskopie verwendet werden, um die häufigen intraartikulären Begleitverletzungen zu adressieren und die Frakturreposition zu unterstützen. Darüber hinaus ist sie bei posttraumatischen Bewegungseinschränkungen sinnvoll einzusetzen. Prospektive Studien zum wissenschaftlichen Nachweis ihres Nutzens stehen noch aus.
Abstract
Background
Hip arthroscopy is an established procedure for the treatment of orthopedic symptoms, such as femoroacetabular impingement. This article describes to what extent this procedure can be sensibly applied within the framework of the treatment of fractures and their sequelae, using the current literature on this subject and own experiences in a major supraregional trauma centre.
Material and methods
A literature search was carried out. The possibilities described in the literature are summarized and additionally, our own experiences are presented. Finally, based on this the risks and benefits of hip arthroscopy in the field of traumatology are analyzed and discussed.
Results
Hip arthroscopy can be used in the field of fracture treatment with acceptable efforts and risks. In general, it provides the possibility to address accompanying injuries for all injury patterns including free joint bodies, cartilage damage and labral tears. In the case of osteosynthesis of the acetabulum, hip arthroscopy enables the removal of interponates and ensures repositioning of the articular surfaces especially in cases where the joint spaces cannot be visualized when anterior approaches are applied. In the case of posttraumatic movement restrictions, hip arthroscopy can be deployed as a minimally invasive treatment of bone or soft tissue impingements.
Conclusion
In addition to repositioning and osteosynthesis, hip arthroscopy can be used to address the frequently occurring accompanying injuries and to support fracture repositioning. Furthermore, it is sensible to used it for posttraumatic movement restrictions. Prospective studies on the scientific evidence of benefits are still to be conducted.
