Aqueductal Stenosis and Hydrocephalus

Abstract

The cerebral aqueduct is a critical structure within the CSF pathway, due to its narrow lumen. Accordingly, aqueductal stenosis (AS) is one of the leading causes of hydrocephalus, though the etiological relationship between AS and hydrocephalus is still far to be clear since AS may also ensue the hydrocephalus. Etiology of AS largely varies from intrinsic obstruction due to several pathological processes to extrinsic compression from a space-occupying lesion of different nature. Moreover, idiopathic form remains frequently encountered.

Radiological imaging is essential to confirm the stenosis, identify the etiology, plan the treatment of AS-related hydrocephalus, and predict the chance of success of the endoscopic treatment.

Indeed, endoscopic third ventriculostomy (ETV) is the gold standard for AS-related hydrocephalus, though some factors as young age and etiology may significantly affect its failure rate. Shunt remains a valid treatment option in infants with unfavorable etiology and when ETV fails to control hydrocephalus.
