Zusammenfassung
Rücken- und Nackenschmerzen gehören zu den am häufigsten angegebenen Beschwerden. Handelt es sich um unspezifische Schmerzen, wird in entsprechenden Leitlinien in den ersten 4 bis 6 Wochen bei fehlender therapeutischer Relevanz keine Bildgebung empfohlen. Finden sich jedoch Zeichen spezifischer Beschwerden infolge ernst zu nehmender Erkrankungen („red flags“), empfiehlt sich eine geeignete bildgebende Diagnostik in Abhängigkeit von der klinischen Verdachtsdiagnose. Das konventionelle Röntgen steht aufgrund der allgemeinen Verfügbarkeit, der leichten Durchführung und der geringen Kosten im Vordergrund, allerdings mit dem Nachteil einer geringen Sensitivität. Bei einer Traumaanamnese, insbesondere nach frischen Traumata, ist die Computertomographie die Methode der Wahl zur Beurteilung der knöchernen Strukturen. Ansonsten ist wegen der besseren Weichteildarstellung die Magnetresonanztomographie vorzuziehen. Besteht der klinische Verdacht auf eine systemische oder multilokuläre Erkrankung, stellt die Skelettszintigraphie aufgrund der hohen Sensitivität und der Ganzkörperdarstellung die günstigste Untersuchungsmodalität dar.
Abstract
Low back pain and neck pain are two of the most commonly reported complaints. In cases of non-specific pain, diagnostic imaging is not recommended within the first 4-6 weeks. If risk factors for serious disorders (red flags) can be identified, further diagnostic imaging is required depending on the suspected underlying pathology. Plain radiography is the most commonly used imaging procedure because of availability and low cost but with the disadvantage of low sensitivity. In cases of traumatic neck pain, computed tomography is the best imaging procedure to exclude bone-related pathologies. In other cases, magnetic resonance imaging is the preferred procedure due to a better visualization of the soft tissue surrounding bone. Bone scintigraphy should be used if signs of a systemic or multifocal disease are present because of the whole body visualization and great sensitivity.
