Abstrait

L’inquiétude augmente face à la progression épidémique de l’obésité dans les pays dont le niveau de vie s’élève, et ce d’autant plus qu’elle concerne désormais toute la population, des plus jeunes aux plus âgés. Dans une société qui vieillit, l’obésité apparaît comme un facteur de risque majeur de morbi-mortalité et comme une cause de fragilité, de handicaps multiples et d’une baisse de la qualité de vie pour la personne âgée (PA) (1-4). Mais quelques paradoxes ont été soulignés : le surpoids est un facteur bénéfique pour l’espérance de vie de la personne âgée dans de nombreuses études (5- 8) et l’obésité peut constituer un facteur de protection contre l’agression en cas de maladie sévère cachectisante (2, 4). De plus, à l’excès de tissu adipeux s’associe souvent une sarcopénie, autre facteur majeur de fragilité pour la personne âgée. Le clinicien est donc amené à prendre en compte le surpoids et la diminution de la masse maigre dans son évaluation nutritionnelle de la personne âgée.
