Summary
Several groups have investigated the pharmacokinetics of cefoperazone in healthy volunteers and have compared them with those of both first and second generation cephalosporins as well as with cefotaxime, the new third generation cephalosporin. Intramuscular administration resulted in high serum concentrations, i. e. for 0.25 to 1 g doses, 22 to 67 mg/l at one hour, which were maintained for up to eight hours (2.1 to 5.6 mg/l). The half-life was 108 to 154 minutes. Urinary recovery ranged from 14% to 18% of the administered dose. Serum levels 15 minutes after a bolus injection (three to five minutes) were 76 to 244 mg/l for doses ranging from 0.5 to 2 g. At eight hours, cefoperazone concentrations were still 2.4 to 11.8 mg/l. The serum half-life (t 1/2) was 115 to 120 minutes and urinary recovery 29% to 33%. Blood levels were dose-related giving peaks five minutes after the injection ranging from 244 to 375 mg/l with 2 g i. v. and 518 mg/l with 3 g. Studies with cefoperazone given by i. v. infusion have given comparable data. Comparative studies have shown that cefoperazone produces higher serum levels which are maintained longer than with cefamandole or cefotaxime, and levels comparable to cefazolin.
Zusammenfassung
Verschiedene Gruppen arbeiteten über die Pharmakokinetik von Cefoperazon bei gesunden Probanden und verglichen die Ergebnisse mit denen von Cephalosporinen der ersten und zweiten Generation sowie mit Cefotaxim, einem neuen Cephalosporin der dritten Generation. Nach intramuskulärer Verabreichung kommt es zu hohen Serumkonzentrationen, d. h. bei Gabe von 0,25 bis 1 g i. m. sind nach einer Stunde Spiegel von 22 bis 67 mg/l nachweisbar, nach acht Stunden noch zwischen 2,1 und 5,6 mg/l. Die Halbwertszeiten liegen bei 108 bis 154 Minuten. Die Urinausscheidung beträgt 14% bis 18% der applizierten Dosis. 15 Minuten nach einer Bolus-Injektion von Cefoperazon über drei bis fünf Minuten betrugen die Serumspiegel nach Applikation von 0,5 bis 2 g 76 bis 244 mg/l. Nach acht Stunden lagen die Serumspiegel noch bei 2,4 bis 11,8 mg/l. Die Serum-Halbwertszeit (t 1/2) wurde mit 115 bis 120 Minuten veranschlagt, die Ausscheidung im Urin betrug 29% bis 33%. Die Serumspiegel waren dosisabhängig und ergaben nach 2 g i. v. Spitzenspiegel zwischen 244 bis 375 mg/l, nach 3 g 518 mg/l, jeweils fünf Minuten nach Injektion. Untersuchungen über Cefoperazon als intravenöse Infusion erzielten vergleichbare Daten. Vergleichende Untersuchungen zeigten, daß mit Cefoperazon, ähnlich wie mit Cefazolin, höhere und länger anhaltende Serumspiegel erzielt werden als mit Cefamandol und Cefotaxim.
