Anorektale Abszesse und Fisteln

Zusammenfassung

Anorektale Abszesse und Fisteln gehören zu den häufigsten Erkrankungen der Analregion. Synonym werden sie auch als Perianalabszess bzw. Analfistel bezeichnet. Für die anorektalen Fisteln liegt die Inzidenz in Europa bei 20 pro 100.000 Einwohner. Der Altersgipfel für anorektale Abszesse findet sich zwischen 20 und 40 Jahren, für Fisteln zwischen 20 und 50 Jahren, dabei sind Männer 3-mal häufiger betroffen als Frauen. Die Rezidivrate liegt in Abhängigkeit der Ausdehnung und Therapie bei den Fisteln zwischen 10 und 50 %.

Da anorektale Abszesse häufig in der Ausbildung anorektaler Fisteln resultieren, sind beide als ein Krankheitsbild zu betrachten. Dabei ist der Abszess als die akute und die Fistel als die chronische Verlaufsform ein und derselben Erkrankung zu werten. Sie haben eine identische Ätiologie und identische Wege, sich in der Anorektalregion auszubreiten. Ihre Einteilung berücksichtigt ihren Bezug zur Sphinktermuskulatur. Ihre Therapie sollte jedoch getrennt betrachtet werden.
