Therapie der Anastomoseninsuffizienz in der onkologischen Kolonchirurgie

Zusammenfassung

Die Therapie der Anastomoseninsuffizienz (AI) nach kolorektalen Resektionen wird wesentlich durch eine zeitnahe Diagnostik und das Beherrschen komplexer chirurgischer, interventioneller und endoskopischer Maßnahmen geprägt. Neben den bekannten Faktoren, die das therapeutische Vorgehen beeinflussen – Alter und Komorbidität der Patienten, Lokalisation der Anastomose, Intervallzeitraum zur Primäroperation und Ausmaß der entstandenen Leckage – also Faktoren, die primär vom behandelnden Chirurgen nicht beeinflusst werden können, spielen jedoch der Erfahrungsstand des Chirurgen und die interdisziplinäre kooperative Therapieplanung und -durchführung eine nicht unerhebliche Rolle. Die aufgezeigten Algorithmen für 3 grundsätzlich unterschiedliche klinische Szenarien sollen helfen, Therapieentscheidungen in diesen Problemsituationen auf derzeit aktuellem Stand des Wissens führen zu können.
