Zusammenfassung
In einer vergleichenden Untersuchung soll der Einfluß der Hornhautkrümmung und der Hornhautdicke nach refraktiver Chirurgie auf die Applanations- und Kontakttonometrie gezeigt werden. Es wurden 36 Patienten nach radiärer Keratotomie und 32 Patienten nach Excimer-Ablation untersucht. Als Kontrollgruppe dienten 40 Normal-patienten ohne vorausgegangene Operation. Die Tensionswerte aller Patienten wurden jeweils mit dem Applanationstonometer nach Goldmann, dem Handapplanationstonometer nach Draeger und dem elektromechanischem Kontakttonometer „Tonopen“ gemessen. In der Kontrollgruppe kann gezeigt werden, daß das Handapplanationstonometer nahezu identische Werte wie das Goldmann’sehe Tonometer mißt. Das Tonopen mißt im Mittel 1 mmHg unter dem Referenzgerät von Goldmann.
Bei zunehmendem Krümmungsradius und dünner werdender Hornhautdicke kehrt sich die Meßgenauigkeit zugunsten des Tonopens zum Gegenteil. Bei der immer populärer werdenden refraktiven Hornhautchirurgie höhergradiger Myopien ist dies vor allem bei der Applanationstonometrie zu berücksichtigen.
Summary
To evaluate the influence of increased or decreased corneal thickness and corneal curvature on applanation tonometry 40 normal subjects, 32 patients after photorefractive keratectomy and 36 patients after radial keratotomy were measured with Goldmanns and Draegers applanation tonometer and with the Tonopen. In corneas with a flatter curvature the desired applanation diameter is reached earlier than in steeper curvatures. Thin corneas will result an underestimation and thick corneas will result in an overestimation of pressure readings. In general tonometry readings are lower after myopic refractive procedures compared to base line data collections.
In consequence the Tonopen should be used for flattened corneas. For higher corneal cuvature-readings this technique gives more reproducable results of the intraocular pressure than applanation tonometry.
