Zusammenfassung
Hintergrund
Mit dem Kerndatensatz „Notaufnahme“ der DIVI liegt ein nationaler Standard zur Dokumentation der frühen innerklinischen Versorgung von Notfallpatienten vor. Ziel des Projekts war es, ein Dokumentationssystem für die Notfallaufnahme am Bundeswehrkrankenhaus Ulm zu entwickeln, welches den Kerndatensatz „Notaufnahme“ der DIVI beinhaltet, aber auch klinikspezifische Gesichtspunkte berücksichtigt.
Methodik
Entwicklung eines Dokumentationssystems untergliedert in 3 Phasen:

Entwicklung und Umsetzung des Systems,

praktische Erprobung in der klinischen Routine.
Als Dokumentationsform wurde die „digitale Papier- und Stifttechnologie“ gewählt. Das modulare Konzept der DIVI wurde an die eigenen Bedürfnisse adaptiert.
Ergebnis
Im Zeitraum von 08/2010 bis 06/2011 wurden 23.549 Patienten (männlich: 51,7%; Alter: Bereich 1,5–92 Jahre) mit dem System erfasst. Die Module gliedern sich wie folgt auf: Basismodule n = 23.549, Konsilmodule n = 4294, Überwachungsmodule n = 1586 und Traumamodule n = 795. Bei mehr als 80% der Patienten war die Dokumentation mit einem einzigen Formular (Basismodul) zu bewerkstelligen. Das System erwies sich als äußerst robust und wurde interdisziplinär wie interprofessionell rasch akzeptiert.
Schlussfolgerungen
Mit Hilfe eines auf digitaler Papier- und Stifttechnologie basierenden Dokumentationskonzepts konnte der Kerndatensatz „Notaufnahme“ erfolgreich in einer zentralen Notfallaufnahme eingesetzt werden.
Abstract
Background
In establishing the core data-set emergency department a national standard of documentation for emergency patients in early hospital treatment has been introduced by the German Interdisciplinary Association of Critical Care Medicine (DIVI). The aim of this project was to create a system of documentation for the emergency department at the Federal Armed Forces Medical Centre in Ulm. This will use the data-set emergency department and is also comprised of additional, hospital-specific details to generate a more extensive external quality management tool.
Results
The development of this system of documentation can be divided in three steps:

The digital pen and paper technology (DPP) was chosen to be the technical platform and is composed of three parts; 1. The actual pen: a slightly oversized biro with a touch sensitive ink cartridge, a CCD camera and a LED diode. 2. The paper: standard paper with a special pattern printed in the background of each form. The pattern allows the pen to recognize its exact position on every form. When something is written, the exact coordinates are recorded along with a time stamp. 3. The verification software: when done with the documentation the pen is put into an USB cradle to synchronize the data with a computer. The coordinates recorded in the software are matched with their related item on the form and then translated into a value in the database. The modular concept created by the DIVI was adapted to fit local requirements. With respect to performing a thorough analysis of process quality the forms trauma and surveillance have been significantly modified. In the period 08/2010–06/2011, 23,549 patients (51.7% male, age 1.5–92 years) were treated and recorded in our accident & emergency department (A&E) using the new system. The forms used were divided as follows: basic form n = 23,549, consultation form: n = 4,294, surveillance form n = 1,586 and trauma form n = 795. This shows that for over 4 out of 5 patients seen in A&E the basic form sufficed. The technical parts proved to be robust and to work reliably. The system was quickly accepted among all users regardless of specialty or profession.
Conclusion
The presented concept of documentation based on the digital pen and paper technology has been approved in routine emergency admissions.
