Zusammenfassung
Bei bestimmten Notfällen können invasive Notfalltechniken lebensrettend sein. Die intraossäre Punktion mit nachfolgender intraossärer Infusion wird eingesetzt, wenn die Etablierung eines intravenösen Zugangs misslingt oder die entsprechende Anlage zuviel Zeit beansprucht und eine Medikamentengabe oder Infusion unerlässlich ist. Eine Notfallkoniotomie wird als „Ultima-ratio-Maßnahme“ dann durchgeführt, wenn weder die endotracheale Intubation noch alternative Methoden der Atemwegssicherung inklusive der Maskenbeatmung gelingen, aber eine zwingende Indikation zur Oxygenierung und Ventilation besteht. Die Anlage einer Thoraxdrainage wird zur Entlastung eines lebensbedrohlichen Spannungspneumothorax eingesetzt. Diese invasiven Notfalltechniken sind sowohl prähospital als auch innerklinisch selten notwendig und werden nur bei bestehender Indikation und vitaler Bedrohung entsprechend nationaler und internationaler Empfehlungen und Leitlinien durchgeführt. Praxisorientierte Ausbildungskonzepte können helfen, den individuellen Anwender zu trainieren und auf die Durchführung dieser nur seltenen aber lebensrettenden Maßnahmen vorzubereiten.
Abstract
Invasive procedures serve as life-saving interventions in certain emergency situations. The intraosseous cannulation with subsequent intraosseous infusion is used in situations where intravenous access is impossible or too time-consuming. An emergent cricothyroidotomy may be performed as a measure of last resort, when tracheal intubation and airway alternatives, including bag-valve-mask ventilation, fail to work, but oxygenation and ventilation are imperative. Chest tube placement is used for life-threatening tension pneumothorax. These indications for invasive techniques are rare, in the prehospital as well as the hospital setting, as they will be performed only in life-threatening situations where national and international recommendations and guidelines justify their use. Practice-oriented educational concepts are useful to provide adequate training to clinicians who may have to perform these rare but life-saving invasive emergency procedures.
