Zusammenfassung
Hintergrund
Im Rahmen der Versorgung nicht rekonstruierbarer Radiusköpfchenfrakturen werden sowohl die Resektion als auch die Implantation einer Prothese beschrieben. In vielen Studien wurden im Zusammenhang mit der Resektion des Radiuskopfes bei osteoligamentären Begleitverletzungen schlechte Ergebnisse angegeben. Aufgrund dieser Erfahrungen wurden verschiedene Prothesentypen entwickelt, die jedoch in der Mehrzahl wegen ihrer materialtechnischen und biomechanischen Schwächen in der klinischen Anwendung nicht überzeugen konnten. Judet entwickelte 1994 eine neuartige bipolare Prothese, welche der natürlichen Anatomie des Radiuskopfes nachempfunden wurde. Ziel unserer Arbeit ist es, die Ergebnisse der bipolaren Prothese von Judet bei nicht rekonstruierbaren Radiusköpfchenfrakturen zu untersuchen.
Patienten und Methode
Zwischen 1995 und 2007 wurden 50 Patienten mit einer Prothese versorgt. Es handelte sich entsprechend der Einteilung nach Mason, modifiziert nach McKee u. Jupiter, um 19 Typ-III- und 31 Typ-IV-Frakturen. 30 Patienten konnten nach durchschnittlich 2,5 Jahren nachuntersucht werden.
Ergebnisse
Anhand des Morrey-Scores ermittelten wir 16 sehr gute, sieben gute, drei mäßige und vier schlechte Ergebnisse. Nach den Kriterien von Radin u. Riseborough hatten 17 Patienten ein gutes, neun ein befriedigendes und vier Patienten ein schlechtes Ergebnis. Radiologisch konnten kein Cubitus valgus, keine Fehlstellung im distalen Radioulnargelenk, kein Ulnarvorschub und keine Osteoporose des Capitulum humeri nachgewiesen werden. Folgende Komplikationen traten auf: eine aseptische Lockerung, eine Prothesenluxation, eine persistierende radiale Gelenkinstabilität, drei Patienten mit Verknöcherungen am Prothesenhals und vier Patienten mit leichter Protrusion ins Capitulum humeri ohne Demineralisierung oder Arthrosezeichen.
Abstract
Background
With regard to the treatment of non-reconstructable radial head fractures, both the resection and the implantation of a prosthesis are considered. Various studies have shown poor results concerning the resection of the radial head with accompanying osteoligamentous injuries. Due to these experiences, different types of prosthesis have been developed. However, the majority of them were not convincing. Judet developed a type of a bipolar prosthesis which had been modeled on the anatomy of the radial head. The aim of our retrospective study consists in examining the results of the Judet prosthesis.
Patients and methods
Between 1995 and 2007, 50 patients were treated with an arthroplasty. These were, corresponding to the classification by McKee and Jupiter, 19 type III and 31 type IV fractures. Thirty patients were available for a follow-up after 2.5 years on average.
Results
According to the Morrey Score, 16 very good, 7 good, 3 fair and 4 bad results could be obtained. Following the criteria of Radin and Riseborough, 17 patients achieved a good, 9 a fair and 4 a bad result. The complications which appeared were: one patient with aseptic loosening, one patient with luxation of a prosthesis, one persistent radial joint instability, three patients with heterotopic ossification and four patients with protrusion relative to the capitulum humeri.
