Zusammenfassung
Es wurden Bestimmungen des intracellulären Säurebasenhaushaltes (pH, pCO2, Standardbicarbonat, Basenüberschuß) an Erythrocyten in Untersuchungen in vitro und bei klinischen Störungen des Säurebasen-haushaltes durchgeführt und den extracellulären Störungen gegenübergestellt. Gleichzeitig wurden der extra- und intracelluläre Na+- und K+-Gehalt bestimmt.
Die Untersuchungen in vitro mit artefizieller Milchsäureacidose, welche durch Zugabe von Na-Bicarbonat oder THAM gepuffert wurde, zeigen Ergebnisse, wie sie an Muskelzellen in situ aus Tierexperimenten bekannt sind. Der intracelluläre pH-Anstieg unter THAM ist größer als der unter Bicarbonat, er tritt jedoch langsamer ein und ist z. T. auf einen Anstieg des intracellulären Bicarbonates unter THAM-Gabe zurückzuführen.
Die klinischen Untersuchungen lassen erkennen, daß extracelluläre Verschiebungen des Säurebasenhaushaltes nicht in jedem Falle gleichsinnige intracelluläre Veränderungen nach sich ziehen. So kann eine extracelluläre Alkalose mit intracellulärer Alkalose, mit intracellulär ausgeglichenem Säurebasenhaushalt oder mit intracellulärer Acidose einhergehen. Das intracelluläre Elektrolytverhalten ist dabei unterschiedlich. Bei extracellulären Acidosen konnte in jedem Falle eine allerdings unterschiedliche große Acidose gefunden werden. Auch in diesen Fällen ist das Elektrolytverhalten nicht einheitlich.
Summary
The intracellular acid-base balance in erythrocytes (pH, pCO2, standard bicarbonate and base excess) was measured in vitro an in clinical disturbances of the acid-base balance and compared with extracellular changes. Concomittantly Na+ and K+ concentrations were determined in erythrocytes and in the serum.
The investigations in vitro with artificial lactate acidosis, which was buffered by sodium bicarbonate or THAM, showed results as they are known from animal experiments with muscle cells in situ. THAM causes a greater rise of the pH in the erythrocytes than sodium bicarbonate, but this rise is more slowly and partly due to an increase of bicarbonate in the erythrocyte during THAM administration.
Our clinical observations show that extracellular changes of the acidbase are not always associated with similar changes in the erythrocytes. Extracellular alkalosis may be associated either with alkalosis, normal acid-base balance or acidosis in the erythrocytes. Concomittant changes of the electrolytes in the erythrocytes are variable. In extracellular acidosis there was always an acidosis in the erythrocytes but of variable degree. Electrolyte changes were also not uniform in this condition.
It is concluded that estimations of acid-base status and electrolyte concentrations in erythrocytes with our simple method increases our insight into the effects of disturbances in acid-base and electrolyte balance and may be valuable in treatment of these conditions.
