74. Probleine bei der Diagnostik des Megacolon congenitum und ihre konsequenzen für die operative therapie

Summary

Hirschsprung's disease must be regarded as an aplasia of the intramural parasympathetic and sympathetic system in the region of the spastic colon segment. In consequence a megacolon develops above the narrow segment and sphincter achalasia in the closure mechanism. The diagnosis can be arrived at in 3 ways. 1. History and rectal examination, 2. X-ray examination, 3. suction biopsy according to Dobbins, with histochemical examination of the acetylcholinesterase activity of the parasympathetic tissue. The suction biopsy sound according to Dobbins can be pushed up into the colon as far as desired, without an anaesthetic and without danger. In the material thus obtained the raised acetylcholinesterase activity can be demonstrated.

Zusammenfassung

Der Morbus Hirschsprung ist als eine Aplasie des intramuralen Parasympathicus und Sympathicus im Bereich des spastischen Dickdarmsegmentes aufzufassen. Als Folge kommt es oberhalb des engen Segmentes zum Megacolon und beim Schließmechanismus zur Sphincterachalasie. Die Diagnose kann auf 3 Wegen erreicht werden: 1. Anamnese und rectale Untersuchung, 2. Röntgenuntersuchung, 3. Saugbiopsie nach Dobbins mit histochemischer Untersuchung der Acetylcholinesteraseaktivität des parasympathischen Gewebes. Die Saugbiopsiesonde nach Dobbins 1äßt sich ohne Narkose und gefahrlos beliebig weit in den Dickdarm hinaufschieben. In dem so gewonnenen Material kann die gesteigerte Acetylcholinesteraseaktivität nachgewiesen werden.
