Abstract

Like many chapters in this text, the origins of the present chapter can be traced to the 1976 discovery of continuous ambulatory peritoneal dialysis (CAPD) by Moncrief, Popovich and their associates [1]. As pediatric nephrologists trained in the 1970’s, the two “senior” authors of this chapter (SRA & JWB) learned the basic techniques of peritoneal dialysis (PD) primarily as a treatment for infants and small children suffering from acute renal failure. Then, as now, PD was widely considered to be the renal replacement therapy (RRT) of choice for acute renal failure in pediatric patients, primarily because PD is intrinsically simple, safe and easily adapted for use in patients of all ages and sizes. However, chronic peritoneal dialysis (CPD) as it was performed in the pre-CAPD era was not very successful in children. During the early 1970’s, hemodialysis for children came into vogue, and we learned to use hemodialysis to treat end-stage renal disease (ESRD) in children. The limitations of available hemodialysis techniques for infants and small children soon became painfully apparent.
