Zusammenfassung

Die chirurgische Therapie muskuloskelettaler Tumoren wird maßgeblich von der Dignität sowie Faktoren wie anatomische Lage, Beziehung zu neurovaskulären Strukturen, Patientenalter, Lebenserwartung und Funktionsanspruch beeinflusst. Die Defektrekonstruktion kann in biologische und endoprothetische Rekonstruktionen unterteilt werden. Modulare Endoprothesen finden aufgrund ihrer freien Verfügbarkeit, Kosteneffektivität und der relativ kurzen Rehabilitationsdauer breite Anwendung, jedoch bei hohen Komplikationsraten. Zu den biologischen Rekonstruktionstechniken, die jeweils individuelle Vor- und Nachteile aufweisen, zählen Allografts bzw. vaskularisierte/nicht vaskularisierte Autografts, Explantation-Replantation (mit Devitalisierung des tumortragenden Segments), Masquelet-Technik und Kallusdistraktion. Indikationsspektrum, mögliche Versagensgründe der einzelnen Methoden sowie Grundsätzen der Behandlung muskuloskelettaler Tumoren werden erläutert.
