Zusammenfassung

Erklärungsmodelle zur Ursache und Entstehung von Fatigue-Syndromen gehen von multifaktoriellen ätiologischen und pathogenetischen Vorgängen aus. Auslösende und aufrechterhaltende Prozesse sowie Schutz- und Risikofaktoren sind daran beteiligt, aber auch Regelungsprozesse von physiologischen Müdigkeits- und Erschöpfungszuständen. Die Tumorerkrankung und die antineoplastischen Therapien spielen bei der Entstehung zentrale Rollen, sowohl als auslösende als auch als aufrechterhaltende Faktoren. Sie können voneinander unabhängig oder gemeinsam wirken und über eine Vielzahl pathophysiologischer Prozesse zum Störungsbild führen. Weiterhin bestimmen erbliche und psychosoziale Faktoren die individuelle Vulnerabilität für Fatigue. Trotz intensiver Forschungsbemühungen und vieler neuer Erkenntnisse ist es bisher nicht möglich, die genauen Anteile der verschiedenen Einflussfaktoren zu bestimmen. Es deutet sich aber an, dass nicht einzelne von ihnen den Entstehungsprozess bestimmen, sondern erst ihre Wechselwirkungen zu den Veränderungen in den Regulationssystemen des Organismus führen, die als Fatiguesymptomatik wahrgenommen wird. Den bisherigen Erklärungsmodellen ist gemeinsam, dass sie noch nicht umfassend empirisch fundiert sind. Dennoch haben sie eine Bedeutung für die Grundlagen und Therapieforschung und einen heuristischen Wert für das Verständnis der Fatigue sowie die diagnostische und therapeutische Praxis.
