Pathogenese, Prophylaxe und Therapie des postoperativen Ileus in der onkologischen Kolonchirurgie

Zusammenfassung

Auch wenn es sich beim postoperativen Ileus meist um ein spontan reversibles Problem handelt, stellt er doch immer eine Herausforderung für das Management dar. Zum einen muss eine mechanische oder septische Ursache ausgeschlossen werden, zum anderem ist eine Aspirationsprophylaxe und Flüssigkeitssubstitution gerade bei älteren Patienten unabdinglich. Inwieweit prokinetische Interventionen sinnvoll sind, ist derzeit noch in der Diskussion. Die Entwicklung eines evidenzbasierten Behandlungsalgorithmus ist besonders vielschichtig, da die Pathophysiologie des postoperativen Ileus sich immer mehr als hochkomplex erweist.
