Zusammenfassung
In früheren Studien unserer Abteilung konnte gezeigt werden, daß eine Herzinsuffizienz aufgrund exzessiv hoher Spiegel endogener Katecholamine von einer Abnahme der Anzahl der sarkolemmalen ß1-Adrenozeptoren begleitet ist, während das myokardiale H2-Rezeptorsystem funktionsfähig bleibt. Der erste klinisch getestete spezifische H2-Agonist, Impromidin, erwies sich als ein potenter Stimulator bei Patienten mit Herzinsuffizienz, deren Myokard nicht mehr auf Katecholamine reagierte. Trotz der insgesamt günstigen hämodynamischen Wirkungen der H2-Rezeptorstimulation war aufgrund einer Reihe unerwünschter Substanzeffekte eine breite klinische Anwendung nicht möglich. Einige neu entwickelte potente H2-Agonisten mit zusätzlichen H1-antagonistischen Eigenschaften (Arpromidin und die difluorierten Analogverbindungen BU-E-75 und BU-E-76) wurden in vitro und in vivo am Meerschweinchen unter physiologischen und unter pathophysiologischen Bedingungen gegen Impromidin als Referenzsubstanz untersucht. Am isoliert perfundierten Herzen waren alle drei neuen H2-Agonisten potenter als Impromidin in der Steigerung der Kontraktionskraft und des Koronarflusses bei gleichzeitig geringer positiv chronotroper Wirkung und wesentlich reduzierten arrhythmogenen Eigenschaften. In vivo wurden qualitativ ähnliche Ergebnisse bezüglich LVdp/dt, LVP, Herzminutenvolumen und Blutdruck ermittelt. Allerdings waren BU-E-76 und BU-E-75 wesentlich potenter als Arpromidin und Impromidin. Die neuen Substanzen zeigten auch hier ein geringeres positiv chronotropes und arrhythmogenes Potential. Am Modell der Vasopressin-induzierten akuten Herzinsuffizienz normalisierten BU-E-75 und -76 im Unterschied zu Arpromidin und Impromidin alle Kontraktilitätsparameter. Aufgrund ihrer kombinierten positiv inotropen und vasodilatatorischen Eigenschaften und ihres im Vergleich zu Impromidin und konventionellen Katecholaminen günstigeren kardiovaskulären Wirkprofils könnten solche H2-Agonisten möglicherweise die Behandlung der Herzinsuffizienz um eine neue therapeutische Alternative bereichern.
