Emergency Management Hiatal Hernia and Gastric Volvulus

Abstract

Gastric volvulus secondary to paraesophageal hernia is rare. This secondary type of gastric volvulus associated with paraesophageal hernia is usually seen in the adults. There is often atypical clinical presentation and low index of clinical suspicion for diagnosis. The gastric volvulus is a grave surgical emergency and deems mandatory prompt diagnosis. Surgical management forms cornerstone in acute gastric volvulus complicated by paraesophageal hernia. The principle of the surgical intervention is to relieve the obstruction, restore the normal anatomical position of the stomach, and prevent recurrent volvulus by correcting the predisposing factors with gastropexy. Active resuscitation with intravenous fluids, nasogastric decompression, correction of electrolyte imbalance, and use of broad-spectrum antibiotics is to be done before any intervention. Open, laparoscopic, or laparoendoscopic technique is available for emergency surgical intervention depending on expertise of surgeon.
