Abstract
The aim of this study was to determine the state of research on long-term sequelae of the shoulder joint of former elite athletes.
A systematic query in PUBMED was carried out using predefined search words and the result lists were looked for studies dealing with long-term sequelae of the shoulder joint of former athletes.
Among over 1000 scanned publications only two studies dealt with long-term changes of the shoulder of former elite (overhead) athletes.
One study showed long-term changes in shoulder of former elite javelin throwers. The shoulder of the throwing arm was more often painful and presented more radiologic signs of degeneration in comparison with the opposite shoulder. Another study demonstrated a higher prevalence of glenohumeral osteoarthritis in the dominant shoulder of former elite tennis players than in sedentary controls.
The number of studies dealing with long-term sequelae of the shoulder joint of former elite athletes is rare. The very few existing data indicate that former overhead athletes may have higher risks of suffering from pain and degeneration of the dominant shoulder joint than the normal population.
Zusammenfassung
Durch eine systematische Literaturanalyse sollte festgestellt werden, welche Daten bislang über Spätschäden am Schultergelenk von Leistungssportlern existieren.
Es erfolgte eine Datenbankrecherche mit vorher definierten Suchbegriffen im PUBMED. Es fanden sich aus über 1000 Publikationen nur zwei Artikel, die explizit Spätschäden des Schultergelenkes bei (Hochleistungs-)Sportlern untersucht haben.
In einer Studie wurde nachgewiesen, dass ehemalige Hochleistungsspeerwerfer im Bereich der Wurfschulter im Vergleich zur Gegenseite auch Jahre nach der Hochleistungsphase deutlich häufiger Beschwerden haben und dass die dominante Seite klinische und radiologische Zeichen einer dauerhaften degenerativen Veränderung zeigt. In der zweiten Studie wurde die dominante Schulter von beschwerdefreien, ehemaligen professionellen Tennisspielern radiologisch untersucht und mit einer Kontrollgruppe verglichen. Es zeigte sich bei den ehemaligen Leistungsathleten eine signifikant höhere Rate von radiologischen Zeichen einer Omarthrose im Vergleich zur Kontrollgruppe.
Die Anzahl der Studien, die sich mit dem Thema Spätschäden am Schultergelenk beim Leistungssportler beschäftigt haben, ist in Relation zu der Vielzahl von Publikationen über Behandlungsmethoden bei sportlich bedingten Verletzungen des Schultergelenkes verschwindend gering. Entsprechend ist die Datenlage dünn. Die wenigen existierenden Studien liefern Hinweise dafür, dass ehemalige Überkopf-Hochleistungssportler nach Beendigung ihrer aktiven Zeit zu einem relevanten Anteil an Beschwerden des Schultergelenkes leiden. Zudem ist eine erhöhte Arthroserate bei diesen Sportlern im Vergleich zur Normalbevölkerung anzunehmen.
