Résumé

Les exentérations pelviennes pour récidives extraluminales de cancers du rectum peuvent bénéficier d’une chirurgie curative mais au prix d’une forte morbidité et d’une qualité de vie altérée. La sélection des patients est le facteur essentiel de « curabilité ». L’IRM pelvienne est l’examen de référence pour porter le diagnostic, déterminer l’extension locorégionale et prédire l’importance du geste d’exérèse. Elle assure une bonne concordance radiohistologique excepté pour l’envahissement latéropelvien qui peut être difficile à distinguer de remaniements fibreux séquellaires de chirurgie ou de radiothérapie, entraînant des exérèses mutilantes par excès.

Abstract

Pelvic exenterations for extraluminal pelvic recurrences from rectal cancer afford a high R0 resection rate but the cost to patients in terms of morbidity and quality of life can be high. Therefore careful selection of patients is needed to achieve more curative resections and increase long-term survival rates. Magnetic resonance (MR) imaging may be useful to provide a preoperative roadmap and to distinguish cancer from fibrosis or radiation changes. In some cases of pelvic sidewall involvement, MR is unable to distinguish tumour extension from radiation changes or fibrosis and patients undergo inappropriate extensive surgery.
