Zusammenfassung
Die Narkose im Notarztdienst ist ein seltener, aber zugleich immens wichtiger Bestandteil der präklinischen Patientenversorgung. Die differenzierte Anwendung der Kombination von Analgetika, Sedativa und Muskelrelaxanzien ist in vielen Fällen wesentliche Voraussetzung für das Gelingen der Atemwegssicherung. Dabei unterscheidet sich die präklinische Narkose und Atemwegsicherung in vielen Punkten von der elektiven Situation in der Klinik. Den daraus resultierenden oft erhöhten Schwierigkeiten muss seitens des Notfallteams aus Notarzt und Rettungsassistenten mit hoher Sachkompetenz und routiniertem Vorgehen begegnet werden. Aufgrund der niedrigen Fallzahlen für die präklinische Narkose ist diese Routine nicht allein im Rettungsdienst zu erreichen, sondern muss durch regelmäßige klinische Tätigkeit geschult und aufrechterhalten werden.
Abstract
Rapid Sequence Induction (RSI) of anesthesia in the prehospital setting is a rare but important part of emergency medical service (EMS). The sophisticated use of a combination of analgetics, sedatives and muscle relaxants is essential for a successful airway management. Anesthesia in the prehospital setting differs in many points from the elective in-hospital situation. To deal with the resulting difficulties the complete rescue team (physician and paramedics) needs profound knowledge and a routine skilled approach. Because of the low frequency of prehospital RSI this routine cannot be reached by EMS practice only and regular hospital training is mandatory.
