Zusammenfassung
Die operative Therapie der kniegelenknahen intraartikulären Frakturen ist im Regelfall eine offene Reposition und Osteosynthese („open reduction and internal fixation“, ORIF). In seltenen Fällen kommt eine primäre Endoprothese zur Anwendung. In wenigen kleinen Studien und Kasuistiken wird dabei von guten Resultaten bei älteren Patienten berichtet. Dabei handelt es sich um Eingriffe mit einer hohen Invasivität, und es werden meist Implantate aus der Revisionsendoprothetik angewandt. Von einer primären Endoprothese profitieren vor allem ältere Patienten mit vorbestehender symptomatischer Gonarthrose, mit erheblichen Gelenkdefekten und der Unfähigkeit zur Entlastung. Bei jüngeren Patienten empfiehlt sich die primäre Rekonstruktion, da das Risiko der frühen sekundären Endoprothese in dieser Gruppe eher gering ist.
Abstract
The surgical treatment of intra-articular fractures of the knee joint is usually carried out by open reduction and internal fixation (ORIF). In rare cases, primary total knee arthroplasty can be used. For this procedure a few small studies and case reports have shown good results for elderly patients. It is a procedure with high invasiveness and revision arthroplasty implants are usually used. Elderly patients, particularly those with presymptomatic osteoarthritis, articular defects and the inability to relieve the strain, benefit significantly from primary total knee arthroplasty. In younger patients primary reconstruction is recommended because for this group the risk of early secondary arthroplasty is relatively low.
