Zusammenfassung
Ziel jeder operativen Versorgung einer proximalen Femurfraktur ist eine stabile Osteosynthese, die eine frühzeitige Mobilisierung erlaubt. Bei der Reposition und Frakturstabilisierung müssen insbesondere der Kollum-Diaphysen- und der Antetorsionswinkel beachtet werden, um das funktionelle Zusammenspiel von Hüft- und Kniegelenk nicht zu behindern. Nicht impaktierte Schenkelhalsfrakturen sind hochgradig instabil. Einfache pertrochantäre Frakturen stützen sich nach der Reposition meist so gut ab, dass auch extramedulläre Kraftträger eine volle Belastungsstabilität garantieren können. Mit zunehmend distalem Frakturverlauf und intertrochantärer Trümmerzone kommt es zur Rotationsinstabilität und zur Drehpunktverlagerung des Bruchbereichs nach lateral und kaudal mit Zunahme der dislozierenden Kräfte. Nur diese Frakturen profitieren von einer intramedullären und rotationssicheren Stabilisierung. Die Schenkelhalsfraktur des alten Patienten und pertrochantäre Frakturen mit relevanter Koxarthrose können auch primär mit einer Hüfttotalendoprothese versorgt werden. Aufgrund der erhöhten Komplikationsrate bei instabilen Fraktursituationen sollte in der Regel jedoch zunächst die Osteosynthese Anwendung finden und die Prothese erst nach Frakturheilung implantiert werden.
Abstract
The objective of any surgical treatment of hip fractures should be stable osteosynthesis, allowing early full weightbearing mobilisation of the patient. Particular attention must be paid to the collo-diaphyseal and antetorsion angles in reduction and fracture stabilization in order not to impair the functional interaction between the hip and knee joint. Non-impacted femoral neck fractures are highly unstable. Intracapsular fractures and trochanteric fractures without rotational instability are generally stable after reduction, such that an extramedullary implant can ensure full weightbearing stability. With a more distal fracture course and greater intertrochanteric comminution zone, rotational instability and pivot transfer of the fracture area lateral and caudal ensues with increasing dislocating forces. Only these kinds of fractures benefit from an intramedullary and rotationally stable osteosynthesis. Primary total hip arthroplasty is a potential option for the surgical care of femoral fractures in elderly patients and trochanteric fractures with relevant coxarthrosis. Due to the increased complication rate in unstable fractures, a primary osteosynthesis is recommended followed by total hip arthroplasty following fracture consolidation.
