Zusammenfassung
Unter einer „Palliativen Sedierung“ wird der überwachte Einsatz von Medikamenten mit dem Ziel einer verminderten oder aufgehobenen Bewusstseinslage (Bewusstlosigkeit) verstanden, um die Symptomlast in anderweitig therapierefraktären Situationen in einer für Patienten, Angehörige und Mitarbeiter ethisch akzeptablen Weise zu reduzieren. Ob diese Behandlungsmaßnahme jedoch „ethisch akzeptabel“ ist, hängt unter anderem von den klinischen Rahmenbedingungen und von der konkreten Ausgestaltung der Maßnahme ab. So wird zum Beispiel in Belgien oder den Niederlanden nicht selten mit der Palliativen Sedierung eine gezielte Lebenszeitverkürzung intendiert und dieses Verfahren im Kontext der aktiven Sterbehilfe/Tötung auf Verlangen/Euthanasie diskutiert.
Der folgende Beitrag gibt eine Übersicht über Begriffe und Definitionen und Durchführungsweisen der Palliativen Sedierung, über die bestehenden Verfahrensanweisungen und Leitlinien (und deren teils widersprüchlichen Vorgaben), als auch über die ethischen Implikationen und Probleme der Palliativen Sedierung zum Beispiel mit Blick auf eine bewusst in Kauf genommene (oder gezielt herbeigeführte?) Lebenszeitverkürzung, die mit diesem Verfahren zur Symptomkontrolle unter Umständen verbunden sein kann.
Abstract
Therapeutic (or palliative) sedation in the context of palliative medicine is the monitored use of medications intended to induce a state of decreased or absent awareness (unconsciousness) in order to relieve the burden of otherwise intractable suffering in a manner that is ethically acceptable to the patient, family and health-care providers. ‘Ethical acceptability’ of palliative sedation therapy depends on its clinical standards and organizational framework. For example in Belgium or in the Netherlands, certain types of palliative sedation have been used with the intention to hasten death, and palliative sedation is partly discussed in the context of euthanasia.
This article offers an overview about terms, definitions, concepts and practical implications of palliative sedation therapy with special focus on existing guidelines and ethical issues that should be kept in mind when initiating this ‘last resort’ option of symptom control in palliative care.
