Ziel
In diesem Beitrag wird über die Erfahrungen bezüglich Verträglichkeit und Wirksamkeit einer adjuvanten Interferon-alfa-2b-Behandlung beim Aderhautmelanom berichtet.
Patienten und Methode
39 Patienten (23 Männer und 16 Frauen im Alter von 35–78 Jahren, Durchschnittsalter 56,5 Jahre) mit Aderhautmelanom wurden nach Abschluss der Behandlung ihres Primärtumors einer adjuvanten Therapie mit Interferon alfa-2b (3 Mio Einheiten 3-mal pro Woche subkutan) für 1 Jahr unterzogen. Bei allen Patienten wurde Alter, Geschlecht, Horizontaldurchmesser und Vertikaldurchmesser des Melanoms, Behandlung des Primärtumors, Nebenwirkungen der Interferontherapie und Überleben dokumentiert.
Ergebnisse
Bei 31 der 39 Patienten (80%) wurde die Behandlung über 1 Jahr durchgeführt. Bei 18 Patienten (46%) kam es zum Auftreten von Nebenwirkungen wie Leukopenie, Thrombopenie, Herzbeschwerden, Erhöhung der Leberwerte und Schwindel (WHO Grad I–III) die zu einer Dosisreduktion führten. Bei 8 Patienten musste die Behandlung abgebrochen werden: bei 5 Patienten wegen des Auftretens schwerster Nebenwirkungen und bei 3 Patienten wegen Metastasierung. Weder in der univariaten Analyse noch in der multivariaten Analyse konnte ein verbessertes Überleben durch die Interferonbehandlung gezeigt werden.
Schlussfolgerungen
Eine adjuvante Behandlung des Aderhautmelanoms mit Interferon alfa sollte erst durchgeführt werden, wenn die Frage von Dosis und Administration für das kutane Melanom gelöst ist.
Abstract
Purpose
In this manuscript, the safety and efficacy of adjuvant interferon alfa 2b treatment of uveal melanoma is described.
Patients and methods
A total of 39 patients (23 male and 16 female, mean age 56.5 years, range 35–78 years) with uveal melanoma were treated with interferon alfa 2b, 3 million units three times a week subcutaneously for 1 year after therapy of the primary tumor. In all patients age, gender, primary melanoma data, therapeutic interventions, treatment side effects and outcome were documented.
Results
Of the 39 patients, 31 (80%) finished the treatment as scheduled after 1 year. In 18 patients (46%) the initial dose had to be reduced due to leucopenia, thrombopenia, cardiac symptoms, elevated of liver function or vertigo (WHO grade I–III). In eight patients, therapy had to be withdrawn because of serious side effects (five patients) and the appearance of metastases (three patients). Neither a univariate approach nor a multivariate approach could show a protective effect of interferon treatment on survival.
Conclusions
Adjuvant treatment of uveal melanoma with interferon alfa should be abandoned until the question of dose and administration for cutaneous melanoma is solved.
