History

A 67-year-old woman presented with a 2-month history of worsening right thumb pain, swelling, and nail pigmentation. The patient is white (non-Hispanic) and a retired office worker. She has a history of fibromyalgia and spinal stenosis with a recent history of lumbar fusion. There was no history of fever, trauma, or any underlying inflammatory arthropathy (Figs. 1, 2, and 3).
