Zusammenfassung
Hintergrund
Markraumeröffnende Verfahren finden seit mehr als 6 Jahrzehnten Anwendung in der chirurgischen Behandlung chondraler Defekte des Gelenkknorpels. Präklinische Untersuchungen haben in den vergangenen Jahren dazu beigetragen, diese Techniken weiter zu optimieren.
Fragestellung
Aktuelle Empfehlungen zur Durchführung markraumeröffnender Verfahren werden anhand der präklinischen Datenlage dargestellt.
Methode
Die vorliegende Literatur wird ausgewertet und diskutiert.
Ergebnisse
Markraumeröffnende Verfahren umfassen die Mikrofrakturierung, subchondrale Anbohrung und fokale Abrasion. Sie resultieren in der Bildung eines fibrokartilaginären Reparaturgewebes. Vor jeder Markraumeröffnung ist ein Defektdébridement, u. a. mit kompletter Entfernung des kalzifizierten Knorpels erforderlich. Zur Perforation der subchondralen Knochenlamelle durch Mikrofrakturierung sind Ahlen mit kleinem Durchmesser und dünner Spitze geeignet, um ein tiefes Eindringen bei gleichzeitig geringer Schädigung des subchondralen Knochens zu gewährleisten. Die subchondrale Anbohrung erfolgt durch Kirschner-Drähte oder Bohrer unter Kühlung. Zur fokalen Abrasion sind Kugelfräsen zweckmäßig. Matrixaugmente können eine sinnvolle Ergänzung darstellen.
Schlussfolgerung
Die präklinische Studienlage zeigt, dass markraumeröffnende Verfahren insbesondere dann in einer zufriedenstellenden Knorpelreparatur resultieren, wenn chirurgisch-technische Parameter wie Eindringtiefe, Durchmesser oder dreidimensionales Design der verwendeten Instrumente optimiert sind.
Abstract
Background
Bone marrow stimulation procedures have been applied for the surgical treatment of chondral defects of articular cartilage for more than 60 years. Recent preclinical investigations resulted in further optimization of these techniques.
Objective
Presentation of the current recommendations for the surgical performance of bone marrow stimulation techniques based on preclinical data.
Methods
The currently available literature is evaluated and discussed.
Results
Bone marrow stimulation procedures, such as microfracturing, subchondral drilling, and abrasion arthroplasty result in the formation of fibrocartilaginous repair tissue. Prior to any bone marrow stimulation procedure, meticulous defect debridement with complete removal of the calcified cartilage layer is necessary. Small diameter microfracture awls with a narrow tip ensure sufficiently deep perforation with simultaneous prevention of excessive subchondral bone damage. Subchondral drilling is best achieved using Kirschner wires or surgical drill bits under cooled irrigation. Burrs are suitable for abrasion arthroplasty. Scaffolding augmentation strategies can represent a valuable adjunct.
Conclusion
The preclinical study data suggest that bone marrow stimulation procedures yield the most satisfying results for cartilage repair when technical parameters, such as subchondral penetration depth and diameter as well as the three-dimensional design of the applied instruments are optimized.
