Zusammenfassung
Osteochondrale Läsionen des Talus sind in den letzten Jahren mehr und mehr in den klinischen Fokus gerückt. Dabei muss zwischen der akuten osteochondralen Läsion (OLT) und der Osteochondrosis dissecans tali (ODT) unterschieden werden. Aufgrund der Erweiterung der diagnostischen Möglichkeiten durch die MRT und der sich stetig weiterentwickelnden operativen Möglichkeiten sind die Behandlungsmöglichkeiten im Hinblick auf die Vermeidung einer OSG-Arthrose, die häufig zu einer Arthrodese oder einer endoprothetischen Versorgung führt, deutlich verbessert. Die derzeit meistverwendete Stadieneinteilung nach Berndt u. Harty [7] ermöglicht in Verbindung mit der Beschwerdesymptomatik des Patienten eine stadiengerechte Therapie. Steht in den Stadien 1 und 2 die konservative Therapie im Vordergrund, wird sie durch die operative Therapie beim therapierefraktärem Stadium 2 sowie den Stadien 3 und 4 verdrängt. Die operativen Therapiemöglichkeiten reichen dabei von einer retrograden Anbohrung über die retrograden Entfernung der Sklerosezone mit Spongiosaplastik und der anterograden Kürettage mit oder ohne Fragmentrefixation bis hin zur osteochondralen Transplantation. Zuletzt wurden auch Arbeiten über die Chondrozytentransplantation bei der OLT vorgestellt. In der Zukunft gilt es, die verschiedenen operativen Möglichkeiten in kontrollierten Studien zu untersuchen, um so den betroffenen Patienten die bestmögliche Therapie anbieten zu können.
Abstract
Osteochondral lesions (OLT) of the upper ankle joint have to be differentiated in lesions resulting after an acute trauma and osteonecrosis such as osteochondrosis dissencans. Because of the relation to traumatic distorsion of the upper ankle joint the diagnostic procedure is often delayed or therapy is not sufficient. Reasons for osteochondrosis dissecans are found in elderly traumatic distorsions or by osteonecrosis unknown origin. Differentation of OLT is done by X-ray and at least MRI. Traumatic lesion are mostly located on the lateral talar dome, whereas necrotic lesions are found on the medial dome. Usually the classifikation of OLT is MRI orientated and leeds occording to the stadium in therapeutic consequence. Beyond the konservativ therapy in the first stadiums operative treatment has to be done in many cases. Treatment principles consist of drilling of the subchondral zone, refixation of the vital fragment, debridment of the chondral part, excision of the fragment and at least osteochondral grafting, mosaicplasty or chondrocyte implantation. Studies with a follow up of 12 to 25 month show good results of osteochondral grafting and with a follow up of >4 years show good results in treatment of higher stadiums of OLT, also secondary treatment after failed surgery. This results promise a good chance for reconstructing hyaline cartilage surface in osteochondral lesions of the upper joint.
