Zusammenfassung
Hintergrund
Um den quantitativen Bedarf für neuartige Therapiemodalitäten zur Behandlung chronischer Trommelfellperforationen abschätzen zu können, ist die Bestimmung der Prävalenz dieses Krankheitsbildes notwendig.
Methode
Von 1000 erwachsenen Personen (Alter: 18–93 Jahre; 453 Männer, 547 Frauen) wurden eine genaue Anamnese sowie der beidseitige Trommelfellbefund erhoben.
Ergebnisse
Bei 8 Personen wurde eine einseitige chronische Trommelfellperforation und bei einer weiteren eine beidseitige Perforation nachgewiesen. Für diese 9 Probanden mit chronischer Trommelfellperforation ergibt sich nach Altersstandardisierung eine Gesamtprävalenz von 0,45% (95%-Konfidenzintervall: 0,03–0,87). Eine Tympanoplastik lehnten 8 der 9 Probanden ab.
Schlussfolgerung
Der hohe Anteil an Personen mit chronischer Trommelfellperforation, die nicht operiert werden möchten, verdeutlicht, dass ein großer Bedarf an neuartigen Therapieverfahren besteht.
Abstract
Background
The goal of this study was to evaluate the prevalence of chronic tympanic membrane perforation in the adult population.
Method
The study population included 1,000 subjects (453 males, 547 females) aged 18–93 years. In addition to interview data, ear examinations were performed.
Results
There were eight subjects with unilateral chronic tympanic membrane perforation and a single subject with a bilateral perforation. The age adjusted prevalence of chronic tympanic membrane perforation was 0.45% (95% confidence interval: 0.03–0.87). Eight out of nine subjects disliked tympanoplasty.
Conclusion
The high proportion of subjects with chronic tympanic membrane perforations who do not want to be operated suggests that there is a considerable need for novel therapeutic procedures.

Der Begriff der chronischen Perforation ist im Gegensatz zum angloamerikanischen im deutschsprachigen Raum nicht üblich. Dieser erscheint aber sinnvoll, da im Allgemeinen Befund, Symptomatik und Therapie weitestgehend unabhängig von der Ursache, zum Beispiel Entzündung, akzidentelle oder iatrogene Verletzung, sind. Auch im Tiermodell ist der Begriff der chronischen Trommelfellperforation etabliert [9].
