Tumorkachexie – Eine besondere Entität und deren therapeutischen Folgerungen

Summary.

Cancer cachexia as a syndrome of generalized host tissue wasting results from interactions of tumor, host and tumor-directed treatment. Cancer cachexia may be associated with all terminal cancer stages and is not tumor-specific. Weight loss, functional impairment and deterioration of the quality of life require palliation. A therapeutic algorithm based on therapeutic standards is proposed: appropriate use of parenteral and enteral nutrition, of progesterone analogues and of metoclopramide. Innovative concepts that may enable prophylactic rather than therapeutic interventions are currently under investigation.

Zusammenfassung.

Tumorkachexie ist die Folge kataboler Stoffwechselreaktionen, die durch Wechselwirkungen von Tumor, Organismus und Therapie induziert werden. Sie ist nicht tumorspezifisch, sondern kommt im fortgeschrittenen Stadium aller Tumorerkrankungen vor. Gewichtsverlust mit Funktionseinschränkungen und subjektiver Verschlechterung der Lebensqualität erfordern eine palliative Therapie. Hierzu wird ein Algorithmus angeboten, der auf gesicherten Therapieprinzipien beruht: Parenterale und Sondenernährung sowie Progesteronanaloga und Metoclopramid sind die therapeutischen Standards. Neue Ansätze versprechen insbesondere prophylaktische Möglichkeiten, deren Prüfung aber klinischen Studien vorbehalten bleiben sollte.
