Summary.
Neuroendocrine tumors of the thymus, also known as thymic carcinoids, are rare tumors of the anterior mediastinum. They occur sporadically or in association with the MEN I syndrome. We present five patients (four male, one female; age of first manifestation 19–53 years) who were operated on at our hospital between 1984 and 1995 for neuroendocrine thymic tumors. A hormone-producing tumor presented with Cushing's syndrome in two patients. Two patients had MEN I syndrome. Only in the female patient the primary tumor was confined to the thymus. We found lymph node metastases in three patients and a distant metastasis in one. The neuroendocrine tumors have a high rate of local recurrences and thus we performed 11 operations using a transthoracic or transsternal approach. Since there was no operative mortality and adjuvant therapies are of limited value, we recommend surgery even in case of recurrence.
Zusammenfassung.
Neuroendokrine Tumoren des Thymus, auch als Thymuscarcinoide bekannt, gehören zu den seltenen Tumoren des vorderen Mediastinums. Sie kommen sporadisch oder im Zusammenhang mit dem MEN-I-Syndrom vor. Vorgestellt werden 5 Patienten (4 Männer, 1 Frau; Erstmanifestationsalter 19–53 Jahre), die in den Jahren 1984–1995 wegen eines neuroendokrinen Thymustumors in unserer Klinik operiert wurden. Bei 2 Patienten war der Tumor hormonaktiv und manifestierte sich durch ein Cushing-Syndrom. Bei 2 Patienten lag ein MEN-I-Syndrom vor. Nur bei einer Patientin war der Primärtumor zum Operationszeitpunkt auf den Thymus begrenzt. Es fanden sich Lymphknotenmetastasen bei 3 Patienten und eine Fernmetastase bei einem Patienten. Die Tumoren zeigten eine starke Tendenz trotz radikaler Resektion Lokalrezidive zu bilden. Insgesamt wurden bei den 5 Patienten 11 transthorakale bzw. transsternale Eingriffe durchgeführt. Da kein Patient an den Folgen der Operation verstarb und adjuvante Therapieverfahren von begrenztem Wert waren, empfehlen wir die Resektion auch im Falle eines Rezidivs.
