Zusammenfassung
In einer retrospektiven Studie wurde die Hornhautsensibilität als Parameter der kornealen Reinnervation mit dem elektromechanischen Aesthesiometer nach Draeger untersucht.
An 7 Patienten, 7 Augen, nach Epikeratophakie, 82 Patienten, 156 Augen nach photorefraktiver Keratektomie mit dem Excimer-Laser, 38 Patienten, 56 Augen, nach Keratomileusis in situ und 12 Patienten, 18 Augen, nach Kryo-Keratomileusis wurde die Hornhautsensibilität an 5 verschiedenen Meßpunkten untersucht.
Bei der Keratomileusis in situ und der Excimer-Laser-Ablation erfolgt die korneale Reinnervation am schnellsten. Zwischen 4 und 12 Monaten vergehen nach der Operation, bis eine normale Empfindlichkeit der kornealen Nervenfasern wiedererlangt ist. Nach Kryo-Keratomileusis werden die verschiedenen Schichten der Hornhaut deutlich stärker geschädigt, eine Normalisierung der Sensibilität ist erst nach 2 Jahren zu finden. Die Epikeratophakie-Patienten weisen noch nach 3 Jahren ein asensibles Hornhautzentrum des Spendergewebes auf, was für eine deutlich gestörte Trophik spricht.
Summary
Corneal sensitivity was measured with the Draeger aesthesiometer after different refractive procedures.
We examined sensitivity on 5 different points of the cornea in 7 patients, 7 eyes, after epikeratophakia, 82 patients, 156 eyes, after photorefractive keratectomy, 38 patients, 56 eyes, after keratomileusis in situ, and 12 patients, 18 eyes, after kryo-keratomileusis.
One year after photorefractive keratectomy and lamellar keratomileusis in situ we found normal sensitivity in the center of the cornea. In both procedures a correlation between corneal reinnervation and depth of the dissection was demonstrated. After kryo-keratomileusis the damage on the layers of the cornea is more perceptible. Only after 24 months sensitivity is normal again.
In epikeratophakia even after 3 years the donor tissue is still asensible, the metabolism is reduced.
