Zusammenfassung
Primäre Optikustumore sind selten. Zumeist zeigen diese Tumore ein charakteristisches klinisches Bild. Zur Sicherung der Diagnose sind weitere Untersuchungen, wie die Fluoreszenzangiographie und die Echographie hilfreich. Im Zeitraum von Juni 1994 bis April 1995 konnten in unserer Tumorambulanz drei Patienten mit primären, prälaminären Optikustumoren beobachtet und ophthalmoskopisch, fluoreszenzangiographisch, sowie echographisch untersucht werden.
Bei unseren Patienten handelte es sich um ein 17jähriges Mädchen mit einem kavernösen Hämangiom, einen 58jährigen Mann mit einem Melanozytom und einen 57 Jahre alten Mann mit einem Astrozytom. Alle Fälle waren unilateral und auf den Sehnerven isoliert. Im Fall des Astrozytoms ergab die interne Durchuntersuchung das Vorliegen einer tuberösen Sklerose. Während der Verlaufskontrolle von rund einem Jahr war es bei unseren Patienten zu keiner wesentlichen Befundänderung gekommen.
Die Diagnose dieser seltenen Tumore kann zumeist schon ophthalmoskopisch gestellt werden. Zusätzlich sichern die Fluoreszenzangiographie und besonders die Echographie die Diagnose und ermöglichen eine exakte Verlaufskontrolle. Primäre Optikustumore benötigen meist keine Therapie, da sie nur in wenigen Fällen progredient sind und nur selten Komplikationen verursachen. Beim kavernösen Hämangiom kann es gelegentlich zu Glaskörperblutungen kommen. Das Melanozytom ist vor allem vom peripapillär gelegenen Aderhautmelanomen abzugrenzen. Beim Astrozytom sollte an die Möglichkeit des Vorliegens einer Systemerkrankung gedacht werden.
Summary
Primary optic nerve tumors are rare, but their clinical features are unique. Further examinations, like fluorescein angiography and echography are necessary for a diagnostic work-up. Between June 1994 and April 1995 we observed 3 cases of primary prelaminar optic nerve tumors. Ophthalmological, fluoresceinangiographical and echographical examinations were done.
A 17 year old woman with a cavernous hemangioma, a 58 year old man with a melanocytoma, and a 57 year old man with an astrocytoma are presented. In each case the tumor was unilateral, only involving the optic nerve head. During an average follow-up period of 12 months the tumors remained unchanged.
In these patients, as in others before, ophthalmoscopical findings are characteristic for primary optic nerve tumors. Fluorescein angiography and especially the standardized echography establish the diagnosis and are helpful for accurate follow-up examinations. Usually treatment of primary optic nerve tumors is unnecessary. They remain stable and do not cause complications. A cavernous hemangioma of the optic nerve head may cause vitreous hemorrhage. The most important differential diagnosis to melanocytoma is an infiltrating peripapillary choroidal melanoma. In patients with astrocytoma, a search for systemic associations may be useful.
