Zusammenfassung
Maligne Melanome sind die häufigsten bösartigen Augentumoren im Erwachsenenalter. In dieser Übersichtsarbeit werden ausgewählte Aspekte dieser Entität beschrieben. Es werden drei verschiedene Manifestationen okulärer Melanome unterschieden: Lidmelanome, Bindehautmelanome und uveale Melanome. Die Lidmelanome verhalten sich klinisch und biologisch wie kutane Melanome. Bindehautmelanome sind selten und entstehen aus der primär erworbenen Melanose oder aus Nävi der Bindehaut. Die Therapie besteht aus der Exzision in Kombination mit einer Bestrahlung und einem adjuvanten Therapieverfahren. Das Melanom der Uvea ist der häufigste primäre maligne intraokuläre Tumor bei Erwachsenen. Die Diagnosestellung erfolgt trotz unterschiedlicher Untersuchungsmöglichkeiten primär klinisch. Standardtherapie des Aderhautmelanoms ist die Bestrahlung. Kombinationstherapien einschließlich chirurgischer Exzision kommen in Sonderfällen wie Tumoren mit großer Ausdehnung zur Anwendung. Das Ziel moderner Behandlungsstrategien ist neben der im Vordergrund stehenden Minimierung eines Metastasierungsrisikos der Bulbuserhalt sowie der Erhalt einer bestmöglichen Sehleistung.
