30. Der sekundäre Hyperparathyreoidismus und die autologe Epithelkörperchentransplantation

Summary

Indications, operative technique and results in 15 patients operated on for secondary hyperparathyroidism and renal osteopathy are reported. The main indications were spontaneous hypercalcemia, hypercalcemia after treatment with oral calcium and Vitamin D and severe fibroosteoclasia. After total parathyroidectomy small pieces of parathyroid tissue were transplantated into a muscle of a forearm. In 13 patients relief of bone pain was registered, in all patients the radiological signs of osteopathy improved. The function of the grafted tissue was recorded by direct measurement of PTH levels in the blood drawn from the cubital vein.

Zusammenfassung

Es wird über Indikation, Operationstechnik und Ergebnisse bei 15 Patienten berichtet, die wegen eines sekundären Hyperparathyreoidismus und renaler Osteopathie operiert wurden. Indikation zur Operation waren eine spontane Hypercalcämie, Hypercalcämie unter Behandlung mit Calcium und Vitamin D und schwere Fibroosteoklasie. Nach totaler Parathyreoidektomie wurden Teile von Parathyreoidea-Gewebe in die Muskulatur eines Unterarms implantiert. Bei 13 Patienten kam es zu einer Besserung der Beschwerden, bei allen Patienten gingen die radiologischen Zeichen der Osteopathie zurück. Die Funktion des Transplantates wurde durch direkte Messung des Parathormons nach Punktion der V. cubiti festgestellt.
