Summary
The turnover rate of free fatty acids (FFA) in the plasma and the esterification rate of free fatty acids to plasma triglycerides (TG) were estimated by constant infusion of palmitic acid-9, 10-H3 in 18 normal subjects and 10 patients with nephrotic syndrome (n.s.).
In the patients with n.s. the concentration of TG and cholesterol in the plasma was significantly elevated. The FFA concentration in the plasma was significantly decreased as compared with the control group. Patients with n.s. showed the same turnover rate of plasma FFA as the normal subjects. The fractional turnover rate of FFA in plasma, the esterification rate of plasma FFA to plasma TG and the fractional esterification rate of plasma FFA to plasma TG were significantly increased in the patients with n.s. The fractional turnover rate of plasma TG, however, was significantly lower in the patients with n.s. as compared with the control group.
In the patients with n.s., significant negative correlations could be shown between the concentration of albumin and the turnover rate of FFA in plasma, the esterification rate of plasma FFA to plasma TG, the fractional esterification rate of plasma FFA to plasma TG, and the plasma TG concentration, respectively.
These findings suggest that the reduced capacity for binding FFA as a result of hypoalbuminaemia is responsible for the low FFA concentration in plasma. The increased fractional turnover rate of FFA in the plasma, the increased esterification rate of plasma FFA to plasma TG and the increased fractional esterification rate of plasma FFA to plasma TG can be explained by a low albumin: FFA ratio which indicates a reduced number of binding sites for FFA as a result of the hypoalbuminaemia.
Zusammenfassung
Bei 10 Patienten mit nephrotischem Syndrom (n.S.) und 18 stoffwechseigesunden Personen wurden mittels konstanter Infusion H3-markierter Palmitinsäure die Umsatzrate der freien Fettsäuren (FFS) im Plasma und die Veresterungsrate der FFS des Plasmas zu Plasmatriglyceriden (TG) bestimmt. Die Patienten mit n.S. wiesen eine signifikant höhere TG- und Cholesterinkonzentration sowie eine signifikant niedrigere FFS-Konzentration im Plasma auf als die Kontrollpersonen. Die Umsatzrate der FFS im Plasma war bei den Patienten mit n.S. gleich hoch wie bei der Kontrollgruppe, die „Fractional turnover rate“ der FFS im Plasma, die Veresterungsrate der FFS des Plasmas zu Plasma-TG und die „Fractional esterification rate“ der FFS des Plasmas zu Plasma-TG waren bei den Patienten mit n.S. signifikant höher als bei Normalpersonen. Die „Fractional turnover rate“ der Plasma-TG war jedoch bei den Patienten mit n.S. signifikant niedriger als bei der Kontrollgruppe. Bei den Patienten mit n.S. ließen sich signifikante negative Korrelationen zwischen Albuminkonzentration einerseits und Umsatzrate der FFS im Plasma, Veresterungsrate der FFS des Plasmas zu Plasma-TG, „Fractional esterification rate“ der FFS des Plasmas zu Plasma-TG und der Plasma-TG-Konzentration nachweisen.
Die beobachteten Korrelationen sprechen dafür, daß die verminderte Bindungskapazität für FFS infolge der Hypoalbuminämie für den niedrigen FFS-Spiegel verantwortlich ist. Die erhöhte „Fractional turnover rate“ der FFS, die erhöhte Veresterungsrate von FFS zu Plasma-TG und die erhöhte „Fractional esterification rate“ der FFS zu Plasma-TG können mit einem ungünstigen Verhältnis Albumin: FFS und somit einer verringerten Anzahl von Bindungsstellen für FFS infolge der Hypoalbuminämie erklärt werden.
