Summary
The effect of endogenous secretin released after intraduodenal instillation of 10 mval HCl/10 min and of exogenous seoretin (Boots) 1 U/kg body weight, on exocrine and endocrine pancreatic function was examined in 6 subjects. Although stimulation of exocrine secretion was observed during instillation of HCl as well as after i.v. administration of secretin, a β-cytotropic effect, as estimated from blood sugar and plasma insulin concentrations in peripheral venous blood, was demonstrable only following exogenous secretin. - In 7 subjects, intraduodenal instillation of 2 × 3 mval HCl during intraduodenal glucose loading with 300 ml 5% glucose/ 20 min, did not result in any change of the blood sugar curve and of the insulin values compared with intraduodenal instillation of 2 × 30 ml 0.9% NaCl with the glucose. Lastly, no β-cytotropic effect could be detected following the intraduodenal instillation of 3 ml ether, which stimulated secretion of a pancreatic juice rich in bicarbonate and enzymes. Thus endogenous secretin may be without appreciable physiological significance, with regard to the better assimilation of glucose and increased insulin secretion after oral glucose loading compared with intravenous glucose.
Résumé
La fonction exocrine et endocrine du pancréas a été examinée sous l'effet d'une instillation intraduodénale de 10 mval de HCl/10 min et d'une injection intraveineuse d'une U/kg de sécrétine (Boots) chez 6 personnes. Tandis que l'on pouvait observer une stimulation de la fonction exocrine du pancréas aussi bien pendant l'instillation de HCl que lors de l'administration I.V. de sécrétine exogène, un effet bêta-cytotrope, mesuré d'après la glycémie et la concentration plasmatique d'insuline dans le sang veineux périphérique, n'a pu être démontré qu'après stimulation par la sécrétine exogène. -'instillation intraduodénale simultanée de 2 × 3 mval de HCl et de 300 ml de glucose 5%/20 min chez 7 personnes ne change ni la glycémie, ni l'insulinémie en comparaison avec la même charge de glucose sans HCl. Chez deux personnes l'instillation intraduodénale de 3 ml d'éther, qui stimule la sécrétion d'un suc pancréatique riche en enzymes et en bicarbonate, ne provoque aucun changement de la glycémie, ni de l'insulinémie. Ces résultats ne parlent pas en faveur de l'hypothèse que la libération de la sécrétine par l'intestin joue un rôle important pour l'assimilation plus rapide du glucose et la sécrétion plus élevée de l'insuline au cours d'une charge orale de glucose en comparaison avec une charge intraveineuse de glucose.
Zusammenfassung
Bei 6 Personen wurde die Wirkung einer intraduodenalen Instillation von 10 naval HCl/10 min und 1 E/kg Sekretin (Boots) auf die exokrine und endokrine Pankreasfunktion untersucht. Während eine Stimulierung der exokrinen Pankreasfunktion sowohl während der HCl-Instillation als auch unter exogener Sekretinzufuhr beobachtet werden konnte, war eine betacytotrope Wirkung, gemessen am Blutzucker und an der Plasmainsulinkonzentration im peripheren Venenblut nur nach exogener Sekretinstimulation nachweisbar. - Die intraduodenale Instillation von 2×3 mval HCl während einer intraduodenalen Glucosebelastung mit 300 ml 5%iger Glucose/20 min bei 7 Personen führte zudem zu keiner Steigerung der Glucoseutilisation oder des Insulinspiegels. Nach intraduodenalem Ätherreiz (3 ml), welcher zur Stimulierung eines bicarbonat- und enzymreichen Pankreassaftes führt, konnten keine Zeichen einer betacytotropen Wirkung beobachtet werden, so daß eine endogene Sekretinfreisetzung für die schnellere Glucoseassimilation und größere Insulinsekretion nach oraler Glueosezufuhr im Vergleich zur intravenösen Glucosezufuhr keine größere physiologische Bedeutung haben dürfte.
