Zusammenfassung
Die Pathophysiologie und chirurgisch veränderte Anatomie, die der Wirkweise der Operationsverfahren zugrunde liegt, reicht von der gastralen Volumenreduktion über die sogenannte duodenale Exklusion, bis hin zur Kreation eines Kurzdarmsyndroms. Dabei spielen hierdurch induzierte hormonelle Veränderungen im Magen-Darm Trakt ebenso eine Rolle, wie das nach zentral vermittelte schnelle Sättigungsgefühl durch die Verkleinerung des Magenvolumens. So führt bereits eine geringe Menge Nahrung zur frühen Sättigung und damit zur Gewichtsreduktion, aber gerade die duodenale Exklusion indiziert zudem einen, von der Gewichtsreduktion scheinbar unabhängigen, effektiven therapeutischen Effekt auf einen vorbestehenden Diabetes mellitus Typ 2.
