Summary
Investigations have been carried out in 55 cases of chronic Lupus erythematosus (LE), 5 of 55 cases showed some clinical and histological similarity to the polymorphous light eruptions, and 3 to lymphocytic infiltration; 5 cases had histological features of lymphocytoma. In all these cases the immunofluorescence method demonstrated in vivo bound immunoglobulins and completement in the dermoepidermal junction. The further course was typical of LE.
In all 25 cases of polymorphous light eruptions and in 5 cases of lymphocytic infiltration, as well as in 80 cases of other skin diseases the results were negative.
The authors believe that the immunopathologic studies are at the moment the best diagnostic method in LE.
In clinically unclear cases immunofluorescence is the method of choice in differentiation of lupus erythematosus from polymorphous light eruptions, lympocytic infiltration and lymphocytoma.
Zusammenfassung
Es wurden 55 Fälle von E. chr. untersucht, von denen 5 klinisch und histologisch einer polymorphen Lichtdermatose und 3 einer “lymphocytic infiltration” ähnlich waren; 5 Fälle wiesen histologische Merkmale eines Lymphocytoms auf. In allen diesen Fällen wurde mittels der Immunfluorescenzmethode die Anwesenheit von in vivo gebundenen Immunglobulinen und von Komplement an der epidermo-dermalen Grenze festgestellt. Der weitere Krankheitsverlauf zeigte, daß es sich wirklich um E. chr. handelte.
In 23 Fällen von polymorpher Lichtdermatose und in 5 Fällen von “lymphocytic infiltration” sowie in 80 Fällen anderer Hautkrankheiten fielen die Untersuchungen völlig negativ aus. Die Autoren sind der Meinung, daß die immunpathologische Untersuchung gegenwärtig die beste diagnostische Methode bei E. chr. darstellt. Sie ist in klinisch zweifelhaften Fällen die Methode der Wahl bei Differenzierung von polymorpher Lichtdermatose, “lymphocytic infiltration” und Lymphocytom.
