What is the relevance of various previous damages of the peripheral nervous system to a Thalidomide polyneuropathy?

Zugleich eine klinisch-neuropathologische Differentialdiagnose verschiedener Formen der Polyneuritis-Polyneuropathie

Summary

The question as to whether a preexisting damage of the peripheral nervous system is a possible prerequisite for a Thalidomide-induced polyneuropathy to become manifest was studied in a male patient — 39 years of age at the onset of the disease — who suffered from neuritis and polyneuropathy respectively of variable causes and affecting various areas at greater intervals for 5 years. Despite the concurrence of considerable previous peripheral nerve lesions due to inflammatory, traumatic and toxic causes a nerve damage definitely attributable to Thalidomide failed to develop following the intake of 100 mg Thalidomide daily for 2 1/2 months, corresponding to a total of 6.9 g Contergan.

The notion hitherto held that a preexisting damage of the peripheral nervous system is necessary for a lesion due to Thalidomide to develop therefore appears to be questionable; obviously more specific causes are involved.

Zusammenfassung

An Hand eines klinisch und neuro-histologisch eingehend untersuchten — bei Krankheitsbeginn 39 Jahre alten Mannes — derwährend5Jahren in größeren Zeitabständen mehrere unterschiedlich lokalisierte und verschiedenartig verursachte Neuritiden bzw. Poly-Neuropathien durchmachte, wird der Frage der “Vorschädigung des peripheren Nervensystems” als möglicher Vorbedingung zur Ausbildung einer Thalidomid-Polyneuropathie nachgegangen. Trotz des Zusammentreffens von erheblichen Vorschädigungen des peripheren Nervensystems durch entzündliche, traumatische und toxische Einwirkungen blieb nach 2 1/2 monatiger Einnahme von tgl. 100 mg Thalidomid, was einer Menge von insgesamt 6,9 g Contergan entspricht, eine sichere thalidomidbedingte Nervenschädigung aus. Die bisherige Annahme der Notwendigkeit einer Vorschädigung des peripheren Nervensystems als Bedingung für die Entwicklung eines Thalidomid-Schadens ist demnach in dieser allgemeinen Form zweifelhaft; hier spielen offenbar speziellere Faktoren eine Rolle.
