Zusammenfassung
Die Polydaktylie ist die häufigste kongenitale Fehlbildung. Sie wird in einen tibialen, einen zentralen und einen fibularen Typ unterschieden. Die chirurgische Behandlung sollte eine vollständige und definitive Entfernung der Polydaktylie anstreben. Die Kommissur zwischen 2 oder mehreren Zehen kann bei Syndaktylie komplett oder partiell fehlen. Die knöchernen Phalangen können getrennt, partiell oder komplett fusioniert sein. Meistens sind, im Gegensatz zu den Handsyndaktylien, keine therapeutischen Maßnahmen erforderlich. Der Hallux varus congenitus ist eine seltene Fehlbildung, die sich mit unterschiedlichen klinischen Bildern zeigen kann, die Operationsindikation wird meistens schon im ersten Lebensjahr gestellt. Bei der Makrodaktylie handelt sich um eine seltene kongenitale Fehlbildung, die sich mit einem abnormen Wachstum aller Gewebe in einer Zehe oder in mehreren Zehen darstellt. Es kommt sowohl zu einem abnormen Längen- wie auch Breitenwachstum. Die Behandlung der Makrodaktylien ist sehr anspruchsvoll und nicht standardisiert. Beim Gigantismus zeigt der gesamte Fuß ein Überwuchs, wobei sowohl der Knochen wie auch die Weichteile und ein oder mehrere Strahlen betroffen sein können. Beim Spaltfuß („cleft foot“, „lobster claw“, „split hand/foot malformation“ [SHFM]) fehlen ein oder mehrere zentrale Fußstrahlen. Der Metatarsus adductus (Sichelfuß) zeigt eine Adduktion des Vorfußes gegenüber dem Rückfuß in Höhe des Tarsometatarsalgelenks. Es kommen prinzipiell 3 chirurgische Eingriffe infrage: mediales Release, tarsometatarsale Kapsulotomie nach Heymann und Mittelfußosteotomien. Der Serpentinenfuß („skewfoot“, „Z-foot“) ist durch einen Rückfußvalgus und einen Metatarsus adductus charakterisiert. Die Indikation zur operativen Korrektur wird bei symptomatischen Füßen gestellt. Bei der Mikrodaktylie sind eine oder mehrere Zehen kleiner und nicht proportional zu den anderen Zehen. Bei der Brachydaktylie sind eine oder mehrere Zehen kürzer, wobei eine oder mehrere Phalangen beteiligt sein können. Die Brachymetatarsie oder angeborene Hypoplasie eines Metatarsalknochens gehört zu den longitudinalen Reduktionsdefekten und ist eine seltene angeborene, das Längenwachstum betreffende metrische Fehlbildung mit Störung der Epiphysenentwicklung. Die Klinodaktylie beschreibt eine Achsenabweichung in der anteroposterioren Ebene einer oder mehreren Zehen. Bei Hammerzehen handelt sich um eine Flexionsstellung der Zehen im proximalen Interphalangealgelenk. „Overriding toes“ stellen meistens eine harmlose Deformität dar. Der Digitus quintus varus superductus kann dagegen, abgesehen vom ästhetischen Aspekt, Hautdruckstellen und einen Schuhkonflikt zeigen. Die „eingeschlagene Zehe“ („underriding toe“, „underlapping toe“, „curly toe“) ist eine häufige Fehlstellung, die schon bei Geburt vorhanden ist. Der Hallux valgus interphalangeus (HVI) ist eine häufige Deformität der ersten Zehe, die in Höhe des Interphalangealgelenks lokalisiert ist.
