Abstract

Moyamoya disease is progressive occlusion of bilateral distal carotid arteries with the formation of collateral circulations. There is critical balance between perfusion delay and stress. Although the cause of Moyamoya disease has been still under investigation, there are debates about the most effective treatment method especially between indirect and direct revascularization. Ischemic attacks and hemorrhage are two major problems of this complicated disease. Revascularization is one of the methods of choice to prevent further ischemic insults but still debating its efficacy on hemorrhagic attack. Repeated ischemic and hemorrhagic insults will accumulate damages in the brain and deteriorate the neurological and functional status of Moyamoya patients. Due to its progressive nature, early diagnosis and timely management for repeated attack, whether it could be ischemic or hemorrhagic, are tremendously important. The critical treatment for the cause of Moyamoya disease hopes to come soon.
