Special Considerations for the Pulmonary Hypertension Patient

Abstract

Practitioners who care for patients with pulmonary hypertension (PH) are often asked to provide expert opinion concerning the optimal management of PH patients in high-risk clinical scenarios. It is imperative that the clinician anticipates the potential complications associated with these events and to provide a comprehensive treatment plan that can be implemented immediately in response to life threatening complications. In this chapter, we will discuss three clinical scenarios that require special consideration as they can be associated with rapid hemodynamic changes that may compromise the capacity of the PH patient to mount an appropriate compensatory physiological response: exercise, pregnancy and surgery. We provide information concerning the physiological changes associated with each of these scenarios and discuss relevant clinical data that will help the practitioner develop a sound treatment plan to effectively deal with specific complications potentially arising in each of these clinical scenarios. It is our goal that this information will help the practitioner institute sound guidelines that will result in improved clinical outcomes for patients with PH.
