Zusammenfassung
Endokrine Tumoren des Pankreas leiten sich von dem diffusen neuroendokrinen System der Pankreasregion ab. Im Angloamerikanischen ist die Bezeichnung Inselzelltumor oder Inselzellkarzinom gebräuchlich. Als Oberbegriff wird nach der WHO-Klassifikation von der neuroendokrinen Neoplasie des Pankreas gesprochen, die anhand des histologischen Grading in die Gruppen gut differenzierter neuroendokriner Tumor G1 und G2 und schlecht differenziertes neuroendokrines Karzinom G3 eingeteilt wird. Handelt es sich bei den neuroendokrinen Karzinomen stets um maligne, schnell wachsende Neoplasien, können gut differenzierte neuroendokrine Pankreastumoren (pNET) klinisch benigne sein oder zu einem metastasierten Tumorleiden mit in der Regel langsamer Wachstumsdynamik führen. Unter klinischen Gesichtspunkten ist die Einteilung in funktionell aktive Tumoren und funktionell nicht aktive Tumoren sinnvoll. Der Ablauf der diagnostischen Schritte wird maßgeblich durch die klinische Situation bestimmt. Bei Verdacht auf ein bestimmtes Hormonsyndrom erfolgt zunächst die Sicherung des Syndromes bzw. der Ausschluss desselben durch eine spezifische Labordiagnostik, gefolgt von Lokalisationsdiagnostik und Staginguntersuchungen. Hingegen wird bei der Abklärung unspezifischer Abdominalbeschwerden wahrscheinlich eine abdominelle Bildgebung und am häufigsten der transabdominelle Ultraschall der erste diagnostische Schritt sein. Therapieentscheidungen sollten bei diesen seltenen und heterogenen Tumoren mit multimodalen Therapieoptionen in einem erfahrenen interdisziplinären Team gefällt werden.
