Abstract
Transplant surgery is by its very nature a medical home to its patients. The transplant team adheres to many of the same principles that the primary care and medical specialties need for their recognition by national organizations, such as first-contact, continuous, coordinated care and an emphasis on excellent patient outcomes. But there is no such recognition yet available under the current definitions of a Patient-Centered Medical Home or Patient-Centered Specialty Practice for surgeons of any specialty. This chapter will discuss the importance and history of the Patient-Centered Medical Home, its evolution to the Patient-Centered Specialty Practice, and the groundwork and effort that will be needed for transplant surgery to be recognized for the similar work it has been doing for years.
