Abstract
The basis of the current classification of testicular germ cell tumors was established by the work of Friedman and Moore, who reviewed almost 1000 cases of testicular cancer and published their findings in 1946 (Friedman and Moore 1946). Refinements to their classification were based on observations of many but by Teilum in particular. Mostofi and Sobin, with the assistance of a distinguished group of pathologists, created the World Health Organization classification in the 1970s, the preferred classification in the United States. PUGH and his colleagues established a somewhat comparable classification, the British Testicular Tumor Panel classification. Skakkebaek and colleagues, through numerous publications, have demonstrated the importance of carcinoma in situ, or intratubular germ cell neoplasia, defining the pathogenesis of germ cell tumors. The classification of germ cell tumors created a framework to evaluate therapeutic strategies, leading to a dramatic decrease in the mortality among patients with a highly lethal tumor. Additionally, the work has led to a histogenetic scheme that has provided the framework to formulate hypotheses that can answer the many remaining questions concerning testicular cancer (Leahy 1992; Ginsburg 1997).
