Samenvatting

Epilepsie is een veelvoorkomende neurologische aandoening die met medicatie veelal goed te behandelen is. Wanneer epilepsie echter niet goed op medicatie reageert (bij ongeveer een derde van de patiënten) worden andere behandelmethoden overwogen. In specifieke gevallen kan dan diepe hersenstimulatie ('deep brain stimulation', DBS) van de voorste thalamuskern worden overwogen. Dit leverde in een RCT bij 110 patiënten een aanvalsreductie van gemiddeld meer dan 50 % op, met een gunstige uitkomst (>50 % aanvalsreductie) bij 68 % van de patiënten. Bijwerkingen zijn vooral stemmingsstoornissen, geheugenstoornissen en slaapproblemen. Veel is echter nog onbekend over de optimalisatie van de respons op DBS bij therapieresistente epilepsie.
